Terms & Conditions:
* FREE medication packing applies to 12 weeks (3 months) of medication from the date of signing the contract and setting up the service.
* You will need to be taking 5 regular medications subject eligible for PBS payment.
* You will be willing to participate in a medscheck or diabetes medscheck with one of our pharmacists.
* You will need to complete the pharmacy admission form and Dose Administration Aid Service funded by PPA.
* Upon completiong of the three month trial each pack will be charged at $4.50 per pack.
* The meal will no longer be automatically delivered unless you would like us to continue.
* To qualify for the free packing deal the same brands of medication must be used in the medication pack that are used in our Synmed robot in order to qualify for the free packing price.
* One free petite meal from the Farm Door range sent with the webster pack delivery.
* The medication costs will be charged to your personal pharmacy account and the funds deducted at the beginning of each month once you have completed the direct debit form.
* Patient may bring in previously dispensed medication to pack if it is the same brand as those used in the Shortland packing facility.
